Privacy

This notice explains the information used by our planning website and volunteer portal. No Clinic Combinator clinic is open. These are administrative tools, not a patient intake or medical-records service.

Administrative questions only. Do not send medical records, completed patient intake forms, health details or patient information through our forms, email or portal. In an emergency, call 911.

Administrative inquiries

The About form asks for your name, email or phone number, and message. The Start a clinic and Partner forms also ask about your organization, its type or your Georgia city. We use these details to understand and respond to your project inquiry.

Online forms send the information through Web3Forms to admin@cliniccombinator.org. Interacting with a form may load hCaptcha to check for spam before you submit. These services can receive browser, network and interaction information as well as the information needed to handle your request.

The email alternative opens your email app instead of using the form service. Email includes your address and the message or attachments you choose to send. Use only the minimum information needed for an administrative question.

Volunteer portal

An application asks for your name, date of birth, role interest, status, affiliation, address, email and phone. Portal records can include your profile, credential details, training results, availability, assignments, confirmations, recorded hours and notifications. These support application review and volunteer coordination. Independent applications are intended for adults 18 and older.

The portal uses Supabase for sign-in and database services. Sign-in and role-based access govern personal and staffing information. Staffing views can show names, roles and assignments for coordination. Project administrators review applications and operational details.

Notification email uses Resend, which receives the recipient address and message content. Sign-in email is a separate process from operational reminders. SMS is still a work in progress.

Administrative help inbox

Husni Admin handles project questions at admin@cliniccombinator.org. The portal help inbox is for selected administrative conversations and is restricted to active project administrators. Ordinary email importing is currently paused. Its import process excludes attachments; it is not a place for patient information or clinical questions.

Administrators review inbox categories and handling decisions. A category suggestion does not provide medical triage or send a reply. Sending an email does not create an appointment or referral.

Browser storage, quiz and maps

The portal keeps sign-in session information in your browser's local storage. Its demonstration mode uses tab storage for example progress and local storage for guide preferences. Sign out on shared devices. Clearing site storage can remove local settings and require you to sign in again; it does not delete records held by the project or its services.

The planning quiz and income examples run in your browser. Their answers are not sent or saved by the quiz. Printing or saving a planning sheet creates a copy you control.

Opening an interactive map loads Leaflet resources from unpkg and map images from OpenStreetMap. Those requests reveal network information and the map area being viewed. “Use my location” asks for browser permission and centers the map on your location; it is optional. Map labels describe planning examples, not open clinics.

The website host and the services above can receive technical request information, such as IP address, browser details and request time, to deliver and protect their services. Their own privacy information describes their practices.

Keeping, correcting or removing information

There is not yet an automatic project-wide removal schedule. Closing an inquiry or signing out does not delete its records. Copies can remain in the form service, email and portal; removing a sign-in account does not necessarily remove applications, notifications or other copies.

For an access, correction or removal request, email admin@cliniccombinator.org. Describe the type of record and the change you need without sending sensitive documents. An administrator will review the request, the identity information needed, the records held and any need to preserve them before making changes. We cannot promise that one action removes every copy held by a service.
